Job summary

Qualifications

• Experience working with individuals with intellectual and/or developmental disabilities.
• Minimum of one year of supervisory experience.
• Knowledge of DBHDD community provider standards and policies.

Responsibilities

• Provide direct care based on the individual treatment plan.
• Administer medications and provide medical treatments.
• Document changes in individual’s progress or conditions.
• Facilitate transitions to other support systems.
