Job summary

HAN Staff in Victoria, VA is seeking a Licensed Practical Nurse (LPN) to join our correctional care team. You will provide direct patient care under the supervision of a RN or physician, administer medications, monitor vital signs, and contribute to individualized care plans.

The role requires an active VA LPN license, at least one year of recent LPN experience, and current BLS. We offer night shifts, competitive pay, and benefits including health insurance and retirement options.

Qualifications

• Current, valid VA license/certification in good standing.
• 1+ year of LPN experience within the last 3 years.
• Primary Source Verification (PSV) on file from licensing board.
• Current required certifications (BLS, N95 fit test, immunization records, TB test).

Responsibilities

• Provide direct patient care under supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs; report changes in condition.
• Assist with patient assessments and implement care plans.
• Perform wound care, specimen collection, and basic nursing procedures.
• Document patient care accurately in the EMR.
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, therapeutic environment; follow policies and infection control protocols.

Shift Details:

• Nights 8.00pm-8:30am

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.

• Administer medications and treatments as prescribed.

• Monitor and record patients' vital signs and report changes in condition.

• Assist with patient assessments and implement individualized care plans.

• Perform wound care, specimen collection, and other basic nursing procedures.

• Document patient care accurately in the electronic medical record (EMR).

• Educate patients and families on health maintenance and treatment plans.

• Assist with admissions, discharges, and transfers.

• Maintain a safe, clean, and therapeutic environment for patients.

• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted VA license/certification in good standing, as required for the position.

• Minimum of 1+ year within the last 3 years of recent experience as a LPN

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test,).
