Job summary

Empath Health in Venice, FL is seeking a dedicated LPN for Continuous Care on the night shift (8:00 PM – 8:00 AM), PRN/Part-time. You will provide bedside observations, follow the care plan, and report changes to the RN case manager.

• Must be a graduate of an approved nursing program and hold a current Florida LPN license.
• Minimum one year experience in an acute care setting; hospice LPN preferred.

• Provide continuous bedside clinical observations and prescribed care.
• Collaborate with RN case manager and report significant changes in patient condition.
• Accept and process physician orders to maximize patient comfort; assist overnight triage.
