Job summary

Emerald Nursing & Rehab Brookside is seeking a Licensed Practical Nurse to join our Brookside unit in Lincoln, NE. You will provide direct nursing care, assess patient needs, administer medications, and work with a multidisciplinary team to achieve the best outcomes for our residents.

You will participate in care planning, document care accurately, and communicate with residents and families. A strong focus on infection control, safety, and compassionate care is essential in this family-oriented

Qualifications

LPN/LVN license issued in Nebraska (or multi-state).
Graduate from an accredited LPN/LVN program.
CPR/BLS certification required; maintain current status.
Excellent verbal and written communication; collaborate with care team.

Responsibilities

Provide direct nursing care to residents including medications, wound care, and treatments as prescribed.
Monitor vital signs, document progress, and report changes to Director of Nursing.
Assist in developing and implementing individualized care plans.
Maintain accurate documentation and clear communication with residents, families, and team.
Adhere to facility policies, infection control and safety protocols.
Offer emotional support and education to residents and families.
Collaborate with RNs, CNAs, physicians and therapists for coordinated care.
