Avamere Rehabilitation at Ridgemont in Port Orchard, WA is seeking a Licensed Practical Nurse (LPN) for full-time and PRN shifts. You will provide essential nursing care under RN supervision, including vital signs, meds, wound care, and daily living support in a resident-focused setting.

The role requires an active LPN license in Washington, CPR/BLS, and prior long-term care experience is preferred. Competitive wages and a comprehensive benefits package are offered.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, active LPN license in the state.
• CPR/BLS certification required.
• Prefer experience in long-term care, geriatric rehab, sub-acute or critical care.
• Experience with electronic medical records documentation.

Responsabilités

• Provide basic nursing care including vital signs and measurements.
• Administer medications as prescribed under RN supervision.
• Perform wound care and monitor for signs of infection.
• Assist residents with hygiene, dressing, bathing, and mobility.
• Ensure proper nutrition and hydration, assist feeding as needed.
• Document care and communicate changes to the RN.
