Qualifications

• Current, unencumbered LPN license in Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple residents in a fast-paced setting.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care according to the plan of care and physician orders.
• Follow individualized care plans created by the Nurse Manager and team.
• Administer medications and treatments per the plan of care and facility policies.
• Monitor residents, report changes promptly, and respond to clinical needs.
• Maintain accurate, timely documentation per orders and regulatory standards.
• Support a safe, respectful environment for residents and families.
