The Jackson Care Center in Jackson, OH seeks a dedicated Licensed Practical Nurse (LPN) to provide direct patient care under supervision. You will monitor residents, administer medications, assist with daily activities, and follow care plans to support health and comfort.

Responsibilities include accurate documentation, collaboration with RNs and physicians, infection control, and ensuring safety. A high school diploma and an active LPN license are required; new graduates welcome.

Qualifications

• Completion of an accredited LPN/LVN program.
• Current and active LPN license.
• High School diploma or equivalent.
• Experience in healthcare or long-term care preferred; new grads welcome.
• Proficient in documenting care and using EMR systems.

Responsibilities

• Provide basic nursing care to residents, including medications and daily living activities.
• Monitor vital signs and report abnormalities to the supervising RN or physician.
• Maintain accurate patient care documentation and respond to changes in condition.
• Collaborate with RNs, physicians, and other staff for cohesive care.
• Follow infection control protocols and ensure resident safety.
