Avalon Rehabilitation and Healthcare Center in New Jersey is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate team. Per diem opportunities and weekend Baylor availability are offered, with a competitive rate of $30–$39 per hour and same day pay.

Core responsibilities include implementing care plans, administering medications, and collaborating with an interdisciplinary team to deliver safe, person-centered care. NJ LPN license and BLS CPR are required.

Qualifications

• Active NJ LPN license required.
• BLS/CPR certification required.
• Ability to care for multiple residents in a fast-paced setting.
• Strong communication and clinical judgment.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow care plans developed with the Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments per POC and policies.
• Monitor residents' conditions and report changes promptly to supervisor.
• Maintain accurate, timely documentation per orders and standards.
• Support a safe, respectful environment for residents and families.
