Job summary

Community Options, Inc. is seeking a Licensed Practical Nurse (LPN) in Rio Rancho, NM to deliver care in a community setting and follow the nursing process with individuals, families, and health team members while maintaining professional nursing standards.

Starting pay is $70,000 per year with a $5,000 sign-on bonus after 90 days of employment per the Executive Director's approval. A Bachelor's degree or equivalent nursing experience and a valid nursing license are required, along with at least

Qualifications

• Bachelor's degree or equivalent nursing experience required.
• Minimum of one year of nursing experience with individuals with disabilities.
• Valid nursing license and driving license with clean record.
• Ability to calculate medication dosages and manage records.
• Excellent written and verbal communication; teamwork.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by standards of professional practice or law.
• Ensure individuals attend medical appointments per ISP.
• Advise supervisor of concerns related to nursing services.
• Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations.
• Develop and update ISP/PCP with an interdisciplinary team to include concerns, observations, and behavioral developments.
• Train employees on the medical needs of assigned individuals.
• Monitor health and medical conditions of individuals and refer for medical services as needed.
• Monitor the use of medications through medication administration logs.
• Ensure health service supplies are available and replenished as needed.
• Ensure program documentation and billable records are completed accurately and timely.
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
• Participate with individuals in community, social, and recreational activities.
• Participate in circles of support for personal futures planning.
• Follow strict OSHA procedures regarding blood borne pathogens.
• Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation.
