Job summary

Community Options, Inc is seeking a Licensed Practical Nurse (LPN) for Santa Fe, NM to provide care in community-based settings. The role uses the nursing process and involves collaboration with individuals, families, and health team members.

The position includes a $5,000 sign-on bonus after 90 days, and requires a nursing degree or equivalent experience, plus at least one year of nursing experience and a valid driver's license. Training and documentation are essential parts of the job.

Qualifications

• Bachelor’s Degree or equivalent experience in nursing field.
• Minimum of one year of experience providing nursing services.
• Valid driver’s license with a satisfactory driving record.
• Complete all state and agency required training per state guidelines.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their ISP
• Advise supervisor of all concerns and issues related to the provision of nursing services
• Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
• Develop and update ISP/PCP with an interdisciplinary team
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure health service supplies are available and replenished
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network by developing relationships with community members
• Participate with individuals in community, social, and recreational activities
• Follow OSHA procedures regarding blood borne pathogens
• Cooperate with licensee and department staff in inspections
• Additional tasks may be assigned
