Qualifications

• LPN licensure required (Massachusetts).
• Valid driver's license and reliable transportation.
• Knowledge of practical nursing and care for individuals with disabilities.

Responsabilités

• Assess health status of individuals and record health data.
• Administer medications per physician orders and program procedures.
• Incorporate prescribed regimens into the nursing plan of care.
• Provide health teaching to individuals, families, and staff per program.
• Coordinate with other healthcare providers to ensure continuity of care.
• Schedule and accompany individuals to medical appointments as required.
• Assist supervisor with nursing services, policy updates and staff orientation.
