Job summary

Fox Trail Memory Care in South River, NJ is seeking a Community Director (LPN) to lead our team and support residents and families. This is a full-time, on-site role offering $85,000 annually.

You will supervise CMAs, CHHAs, and activities staff, oversee clinical documentation, and ensure compliance with NJ DOH regulations while maintaining an inviting environment for residents and their loved ones.

Qualifications

• Active LPN license in New Jersey.
• Minimum 1 year of experience in senior living, assisted living, memory care, or home care.
• Administrative and/or management experience in a healthcare setting.
• Strong organizational and communication skills.
• Bilingual in Spanish preferred, but not required.

Responsibilities

• Supervise and support CMAs, CHHAs, and activities staff.
• Hire, onboard, train, and coach team members.
• Manage staff schedules to ensure adequate coverage.
• Complete resident assessments at admission, quarterly, and as needed.
• Oversee medication administration and clinical documentation.
• Ensure compliance with NJ DOH regulations and company policies.
• Conduct internal audits and quality checks.
• Manage resident records and staff files.
• Communicate resident condition changes to families and providers.
• Oversee meal supply ordering and home cleanliness.
• Lead monthly special events and support daily activities.
• Serve as the primary point of contact for families.
• Conduct tours and partner with marketing to maintain occupancy.
• Coordinate smooth move-ins and a welcoming environment.
