Zusammenfassung

RiseWell Community Services seeks a dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in West Babylon, NY. This part-time role focuses on community-based support for individuals with serious mental illness, collaborating with psychiatrists and a mobile treatment team.

The position involves medication management, health assessments, and crisis coverage, with duties performed primarily in the community.

Qualifikationen

Licensed Practical Nurse in New York State (NYS) required.
Valid and clean New York driver license.
Minimum 1 year of psychiatric experience in a treatment or rehabilitation setting.
Fingerprinting and background check as part of onboarding.

Aufgaben

Conduct routine mental status exams.
Assess physical health needs of clients.
Refer to physicians and community services as needed.
Medication management and IM injections with psychiatrist.
Participate in a mobile, multidisciplinary ACT team.
Support community-based treatment and crisis coverage.
