Job summary

Community Options, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver care in Rio Rancho, NM. The LPN will assess, plan, implement, and evaluate nursing care within a community setting while coordinating with health professionals and families.

The position requires an LPN license, a valid driver’s license, and at least one year of experience with individuals with disabilities. A $5,000 sign-on bonus is available after 90 days, with starting pay of $70,000 per year.

Qualifications

• Bachelor’s Degree or equivalent experience in nursing field.
• Minimum of one year of experience providing nursing services to individuals with disabilities.
• Valid nursing license.
• Valid driver’s license with a satisfactory driving record.
• Complete all state and agency required training per state guidelines.
• Ability to calculate figures related to medication dosages.
• Understanding and commitment to community-based support for persons with disabilities.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by professional practice or law.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of all concerns and issues related to nursing services.
• Create and distribute medical/health reports detailing general health and physician recommendations.
• Develop and update ISP/PCP with interdisciplinary team.
