Job summary

EverTrue in the United States is seeking a Licensed Practical Nurse (LPN) to provide and coordinate care for residents, following the state Nurse Practice Act and EverTrue policies.

You will administer medications, monitor IV fluids, support an interdisciplinary team, and ensure safety and quality of care. LPN licensure and CPR certification required; gerontological nursing experience preferred.

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practices nursing care within the scope of licensure and state Nurse Practice Act; maintains active license and follows licensure rules.
• Collaborates with interdisciplinary team to define individualized care plans; supports regulatory compliance and staff education.
• Administers medications and treatments; monitors IV fluids and nutrition as ordered.
• May initiate IV therapy if IV-certified.
• Maintains observation, assessment, and documentation per licensure level and care plans.
• Uses electronic medical record for full clinical documentation.
• Observes resident status and coordinates with resident services to ensure continuity of care; documents information timely.
• Maintains safe, comfortable environment; assesses hazards and implements safety measures.
• Oversees and directs certified staff to meet EverTrue standards of care.
• Reports incidents and supports investigative protocols per regulations.
