Job Statement/Summary

The Licensed Practical Nurse (LPN) and Certified Medical Assistant (CMA) support the delivery of direct patient care and communication with patients across Southern West Virginia Health System. Responsibilities include providing direct patient care, patient education, health maintenance and disease management instructions as directed by providers and assisting with follow-up care and care coordination under clinical supervision. Travel to other sites is an expectation for LPNs and CMAs hired in “float” positions and may also be required of other LPNs and CMAs to support operational and patient care needs across the organization. Work schedules may vary based on operational needs and may include extended hours, Saturdays, and designated holidays.

Essential Functions & Responsibilities Of The Position
Primary Care / Acute Care Clinic Assignment
  • Responds to patient telephone inquiries, provides information per inquiry, and accurately reports concerns to the provider.
  • Ensures timely communication of providerreviewed test results and coordinates followup care in accordance with provider instructions and organizational protocols.
  • Instructs patients on health care information as directed by the provider.
  • Assists in processing patient cases and related tasks under provider guidance.
  • Records and completes patient chart documentation as instructed.
  • Assists in retrieving records and reports from consults and referrals.
  • Maintains effective and compassionate communication with patients, families, and staff.
  • Operates standard office equipment including computers, fax machines, and copiers.
  • Assists visitors in the facility by directing or connecting them with the appropriate personnel.
  • Projects a professional image and adheres to all LPCC/SWVHS policies in communication.
  • Assists in confirming and scheduling patient appointments.
  • Completes patient rooming and assessment activities, including collection and accurate documentation of vital signs, medication reconciliations, chief complaint, health history updates, and other relevant clinical information to support quality patient care and provider decisionmaking.
  • Assists with quality measures, reporting requirements and organizational performance initiatives.
  • Assists providers during examinations and procedures.
  • Administers medications, immunizations, injections as ordered and within scope of practice.
  • Performs EKG’s, screenings and other diagnostic procedures as ordered and within scope of practice.
  • Performs point of care testing and collects specimens for laboratory testing.
  • Submits documentation in a timely and accurate manner.
  • Monitors clinical supplies, medications, vaccines, and other patient care materials for adequate inventory levels and expiration dates; removes expired items from use and ensures proper disposal in accordance with organizational policies, manufacturer guidelines, and regulatory requirements.
  • Cleans, disinfects and prepares examination rooms and equipment according to clinical need and infection prevention standards.
  • Maintains assigned clinical controls and quality assurance activities, including but not limited to monitoring and documenting refrigerator temperatures, performing and documenting autoclave spore testing, crash cart/AED checks and other required equipment, safety, and regulatory compliance checks in accordance with organizational policies and applicable standards.
  • Performs weekly checks of all medications.
  • Supports and assists in the training of new employees.Attends required inservice trainings and meetings as assigned by the Regional Nurse Manager.
  • Functions as a member of the patient care team in collaboration with assigned providers. Reports to and receives supervision from the Nurse Manager, who is responsible for staffing, workflow oversight, performance expectations, and interpretation of organizational policies and procedures.
  • Maintains practice within the appropriate scope of licensure/certification and promptly reports any concerns regarding assignments, requests, or duties that may exceed scope of practice or conflict with organizational policies, procedures, or regulatory standards.
  • Safeguards patient privacy by maintaining the confidentiality of medical records, patient information and organizational data, ensuring compliance with HIPAA and all applicable federal, state and organizational requirements.
  • Performs other duties as assigned.
School-Based Clinic Assignment

If assigned to a school-based clinic, the LPN and CMA also perform the following responsibilities:

  • Registers patients.
  • Distributes, explains, obtains, verifies and process patient consent forms and other required documentation, ensuring completeness, accuracy, and compliance with organizational policies and regulatory requirements.
  • Helps to ensure the safety, health, and wellbeing of patients while they are in the clinic.Performs other duties as assigned.
JOB REQUIREMENTS AND PREFERRED QUALIFICATIONS

Education (level and type) and Related Work Experience
  • Graduate from a Licensed Practical Nurse Program or Certified Medical Assistant program.
  • Holds a current unincumbered LPN License or a CMA certification.
  • Possesses basic computer literacy.
  • Demonstrates ability to comply with safety protocols and communicate effectively in English.
