Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to serve as Clinical Coordinator for our Home Health team in Mansfield, LA. You will support clinical staff, coordinate patient care, and handle orders, scheduling, and documentation to ensure quality outcomes.

The role requires an active LPN license in Louisiana, at least 1 year of clinical experience, ability to lift up to 50 pounds, and willingness to travel within the branch area. Bilingual English/Spanish is preferred.

Qualifications

• Licensed Practical Nurse (LPN) license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Ability to travel between branch locations; valid driver's license required.

Responsibilities

• Coordinate patient care with clinical teams and ensure efficient workflows.
• Review and process Homecare Homebase (HCHB) notes and admin tasks accurately.
• Enter physician orders for review and approval by the Clinical Team Manager.
• Monitor the At-Risk patient registry to support quality care.
• Assist patients with scheduling, authorizations, and addressing questions.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify Medicare notices are provided and maintain patient confidentiality.
• Collaborate with staff and case managers for coordinated care.
• Provide direct patient care through field visits as needed.
