On-site Mondays and Wednesdays; remote Tuesday, Thursday, Friday; weekend and holiday rotation. Current LPN license required and minimum 2 years clinical experience, including medical/surgical and community health.

Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers.
On-Site: Monday, Wednesday 8:00AM-4:30PM; VNA Care 67 Millbrook Street Worcester, MA 01606
Remote: Tuesday, Thursday, Friday 8:00AM-4:30PM
Weekend and Holiday rotation
Current LPN License required.
