VNA Care is seeking a Hospice Facility Admission RN for a Worcester hospital-based role, Monday to Friday, 8:00 AM to 4:30 PM. You will lead initial assessments, educate patients and families, and coordinate admission logistics within the hospice program.
The ideal candidate will have Massachusetts licensure, an nursing degree, and at least one year of acute care with hospice or oncology experience preferred. Compensation reflects experience.

Position Summary: The hospice facility admission nurse serves as the first point of contact in the patient’s hospice journey. This role is responsible for the initial education of patients and families entering hospice care and plays a vital role in the understanding and acceptance of hospice services. The hospice admission nurse will conduct comprehensive assessments of patients, facilitating a smooth admission process, and providing compassionate coordination of care.
Compensation based on experience $93,600 - $97,760 per year
