Job summary

LifeCare Medical Center in Roseau, MN invites an LPN to join our team in the Days shift (8am–5pm), 0.8 FTE. The role supports nurses in assessing and documenting patient needs, coordinating care with physicians and the RN charge nurse, and ensuring timely communication with patients and families.

The ideal candidate holds a MN LPN license, has strong communication skills, and can work effectively within a multidisciplinary team.

Qualifications

• current active MN LPN license
• current BLS certification, preferred
• excellent communication and interpersonal skills
• safe medication administration practices
• ability to care for a diverse population of patients with courtesy and respect
• good teamwork and organizational skills
• good decision making and time management skills
• excellent clinical skills

Responsibilities

• Assists the registered nurse or other licensed practitioner in identifying patient needs and goals.
• Contributes to evaluation of the plan of care by documenting patient responses to nursing interventions.
• Demonstrates competency in department standards of care and procedures.
• Functions as a team member contributing to an integrated patient-centered care plan.
• Delegates nursing interventions to trained personnel as appropriate.
• Identifies early warning signs of patient condition changes and documents care.
• Communicates patient information with families and other team members to ensure quality care.
• Participates in discharge planning and patient education as needed.
