Delta Health Center Inc. is hiring for a full-time LPN to triage patients, coordinate referrals to specialists, and manage scheduling with a focus on accuracy and patient care. The role requires IV certification and completion of a Licensed Practical Nurse program.

On-site work with a strong emphasis on independent, detail-oriented patient support. You will handle calls from patients and offices, expedite referrals, and ensure timely addressing of appointment needs while maintaining thorough

Qualifications

• Licensed Practical Nurse (LPN) required or LVN preferred.
• Vigilant in triage and prioritization of patient needs.
• Experience with coordinating referrals and patient communication.

Responsabilités

• Triage Patients and determine care needs.
• Expedite referrals to specialty offices and complete referral requests.
• Answer calls from patients and specialty offices; track referrals.
• Remind patients of upcoming appointments and maintain scheduling reminders.
• Work independently and maintain a high standard of organization and accuracy.
