Job summary

Sparta Health & Rehab is seeking an LPN to join our dedicated team in providing high-quality, person-centered nursing care to residents. You will work closely with RNs and CNAs to deliver meds, monitor conditions, and document care plans, ensuring adherence to all policies.

Qualifications

• Active LPN license in Georgia or eligible.
• Strong clinical judgment and communication skills.
• Experience in long-term care preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
