Job summary

Rockcastle Health & Rehabilitation Center, part of Signature HealthCARE, is seeking a dedicated LPN to provide direct nursing care, supervise nursing staff, and coordinate resident care plans. The role emphasizes quality patient care, documentation, and adherence to long-term care regulations.

Applicants should have an active LPN/LVN license, CPR/BLS certification preferences, and at least one year of nursing experience, preferably in a long-term care setting.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while maintaining comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts according to policies.
• Monitor residents' status and notify physician or responsible party of changes.
• Develop, implement, and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in quality assurance activities and staff education.
