Graceville Rehab is seeking a dedicated LPN Charge Nurse in Florida to lead and supervise nursing care. You will administer medications, monitor residents, and coordinate with the interdisciplinary team to ensure safety and quality of care.

Qualifications

• Active LPN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required.

Responsibilities

• Deliver medications and treatments with supervision of Medication Aids where applicable.
• Monitor resident conditions and respond quickly to changes, ensuring safety, comfort, and continuity of care.
• Support activities of daily living.
• Maintain clear, timely documentation.
• Communicate effectively with the interdisciplinary teams and families.
• Adhere to infection control and safety standards to protect residents and teammates.
• Other duties as assigned.
