Job summary

Villa Maria Nursing and Rehabilitation in Connecticut is seeking an LPN Charge Nurse for full-time shifts 7A-3P and 3P-11P to oversee a nursing unit and ensure quality resident care.

You will mentor LPNs and CNAs, develop care plans with the interdisciplinary team, and maintain accurate documentation while supporting regulatory compliance and quality initiatives.

Responsibilities

• Oversee the daily activities of the nursing unit, ensuring that all aspects of care are delivered in accordance with regulatory standards and Villa Maria Nursing and Rehabilitation's policies and procedures
• Provide guidance, support, and feedback to LPNs, CNAs, and other nursing staff to promote a positive and productive work environment
• Develop and implement individualized care plans for residents, in collaboration with other members of the interdisciplinary team
• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordination of care and effective communication
• Maintain accurate and up-to-date records of resident care, including progress notes, medication administration records, and other relevant documentation
• Participate in quality assessment and improvement activities, including audits, chart reviews, and quality improvement initiatives
