Job summary

Capital Health is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under the supervision of a Registered Nurse. The role involves monitoring patient conditions, administering medications, and assisting with discharge planning in a 36-hour work week.

Qualifications

• Education: Graduate of an accredited Licensed Practical Nursing program.
• Experience: Less than one year related experience.
• Credentials: AHA BLS - Healthcare Provider, Licensed Practical Nurse.
• Knowledge: Proficient in English, verbal and written communication; computer skills.
• Usual Work Day: 12 Hours.

Responsibilities

• Provides direct patient care utilizing the nursing process under the direction of the Registered Nurse.
• Reviews diagnostic test results and reports abnormal results to the RN, physician and healthcare team.
• Collaborates with the RN on development and implementation of the plan of care.
• Administers prescribed medications and treatments per Licensed Practical Nursing standards.
• Maintains and reviews patient records and documents care activities.
• Assists with discharge planning and patient teaching under RN supervision.
• Maintains patient safety and collaborates with the team.
