Job summary

Trilogy Health Services, LLC is seeking a Licensed Practical Nurse (LPN) at The Springs at Lafayette in Lafayette, IN. The role focuses on direct, quality clinical care and supervision of nursing assistants on the unit.

You will observe residents daily, respond to changes in condition, and provide holistic care within the LPN scope while assisting with medication monitoring and administrative duties. Weekends may be required, with competitive benefits offered.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident's physical and emotional status and notify the charge nurse, physician, or Director of Nursing when there is a change in condition.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the LPN scope of practice.
• Assist in monitoring medications, supplies, and equipment to ensure adequate stock.
• Perform administrative duties such as completing forms, reports, evaluations, and charting.
