Job summary

Benchmark Senior Living in Stamford, CT is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, home-like Skilled Nursing setting. You will join a dedicated team and play a key role in assessments, care planning, and daily resident support.

Required are an active LPN license and CPR certification, graduation from an approved LPN program, and 1–2 years of nursing experience, preferably with elderly residents.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program.
• Minimum of 1–2 years of nursing experience, preferably with elderly residents.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports changes in resident health to the Director of Nursing and physicians per state regulations.
• Directs and supervises daily work of Resident Care Assistants.
• Mentors and directs Lead Resident Care Assistants in their duties.
• Coordinates care needs with community providers via case management.
• Balances service demands with resident independence.
• Reports and helps prevent elder abuse, neglect, and exploitation per state regulations.
• Responds to emergencies and uses personal emergency response system and calls for backup when needed.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports any significant changes in resident behavior and health to the Director of Nursing, physician, family, RCAs, and Health Center Administrator per state regulations.
• Directs and supervises the daily work assignments of the Resident Care Assistants.
• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via an effective case management process.
• Effectively balances service demands with supporting resident independence.
• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification
• Graduate of an approved LPN program (per state requirements)
• Minimum of 1 to 2 years of experience working as a nurse preferred
• Previous experience working with the elderly in a paid or volunteer position is highly desirable
