Job summary

Visiting Nurse Health System seeks a Care Manager LPN to assess, coordinate, and monitor community-based services for elderly or disabled clients in Bibb & Peach Counties.

You will conduct in-home assessments, develop care plans, coordinate with physicians, social workers, and family caregivers, and ensure continuity of care across hospital-to-home transitions. Georgia licensure and CPR are required, with 2 years in community health or geriatrics.

Qualifications

• Must hold an active Georgia LPN license and CPR certification.
• Minimum 2 years nursing experience in community health, long-term care, or geriatrics.
• Knowledge of Medicare/Medicaid regulations and care coordination best practices.
• Ability to work independently in a community-based environment with strong documentation.
• Proficient with EMR systems and standard office software.

Responsibilities

• Conduct in-home or facility-based assessments to determine services.
• Develop care paths and discharge plans based on client needs.
