Job summary
Doral Health & Wellness in Brooklyn, NY is seeking a compassionate Licensed Practical Nurse (LPN) to join our Care Management team. The LPN Care Manager will coordinate patient care, monitor member needs, communicate with healthcare providers, and connect patients with appropriate healthcare and community resources.
This is a full-time position offering $38 per hour, 26 PTO days, benefits, and a 401(k). Ideal candidates have NY LPN license, strong communications, and the ability to work
Qualifications
- Current New York State LPN license in good standing.
- Previous nursing experience required.
- Experience in care management or home care preferred.
Responsibilities
- Coordinate and monitor patient care plans and services
- Conduct patient follow-ups and assess ongoing care needs
- Communicate with patients, families, healthcare providers, and community organizations
- Assist with referrals and connections to appropriate healthcare and community resources
- Monitor patient progress and identify changes in health or care needs
- Follow up on appointments, treatments, and recommended services
- Maintain accurate and timely care management documentation
- Collaborate with nurses, providers, social workers, and other members of the care team
- Advocate for patients and help them navigate healthcare and community resources
