Job summary

Signature HealthCARE in Bowling Green, KY is seeking an LPN/LVN to provide direct resident care, supervise nursing assistants, and manage medications and documentation in a fast-paced skilled nursing setting. You will participate in care planning and coordinate admissions and discharges with the interdisciplinary team.

The role requires an active LPN/LVN license, 1 year of clinical nursing experience (long-term care preferred), and familiarity with EHR systems.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physicians of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
