Job description

LPN Care Coordinator - Remote Patient Monitoring (Days)

Schedule: Thursday-Monday, 8:00 AM - 4:30 PM. This is a remote position; only Georgia residents will be considered.

Summary:

The Population Health (PH) LPN Care Coordinator supports departmental goals by identifying and coordinating care for patients involved in the Remote Patient Monitoring (RPM) program, patients discharged from the hospital to home through the Transitional Care Management (TCM) program, and patients needing follow-up care with the Mobile Integrated Health (MIH) team. The role involves working with patients and families to assess care needs, educating them about their health conditions, and collaborating with the MIH Advanced Practice Provider (APP), primary, and specialty care teams to ensure optimal patient outcomes.

Qualifications:

  • Two years of college coursework; Bachelor's degree preferred
  • Licensed Practical Nurse (LPN) certification
  • Three or more years of experience in clinical, community resource settings, care coordination, or case management
  • Experience with EMR and computerized charting systems, such as Epic
  • Strong leadership, communication, educational, critical thinking, counseling, and telephonic management skills
  • Ability to work independently and collaboratively
  • Knowledge of and connections to community healthcare and social resources in Georgia
  • Basic Life Support (BLS) Certification
  • Proficiency in a second language, preferably Spanish
