Job summary

Artis Senior Living of Yardley is seeking a Coordinator of Health & Wellness (LPN/LVN) for the first shift, full-time. This role provides clinical monitoring, medication administration, and care planning for residents with Alzheimer's/dementia in compliance with state regulations.

You will assess residents, supervise CNAs, support move-ins, and engage families. Competitive benefits offered; starting pay is $33/hour with every other weekend required.

Qualifications

• Must possess current LPN or LVN designation in the state.
• Experience with elderly or dementia care preferred.

Responsibilities

• Take an active role in resident assessments, development of service plans, and daily care.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents in accordance with physician orders.
• Document and report care provided and resident observations including changes in condition.
• Provide emotional support and ensure safety of residents.
• Supervise Caregivers and CNAs in providing resident care.
• Ensure high-quality service in a compassionate atmosphere that respects residents' needs.
• Assist with move-ins and relocations, helping residents adjust to the community.
• Participate in community outreach and public relations events as needed.
