Job summary

Sentara Health Plans is hiring a Care Coordinator/Licensed Practical Nurse (LPN) for LTSS Waiver/Home Health in Culpeper and surrounding VA communities. In-person assessments with remote/home office work are required.

The role focuses on EPSDT/member outreach and DMAS-compliant care across multiple counties in Virginia. Education includes an LPN license with VA eligibility; an associate degree is preferred and a bachelor’s in health-related field is acceptable in lieu of nursing education.

Qualifications

• Associate's degree in Nursing preferred.
• LPN license in VA or Compact/Multistate preferred.
• 3 years Managed Care experience required.
• Case Management experience preferred.
• LTSS waiver/Home Health experience preferred.

Responsibilities

• Coordinate EPSDT outreach and ensure DMAS-aligned immunization and well-child exams.
• Enter data and monitor reports to boost EPSDT service utilization.
• Maintain tracking for new enrollees and adherence to immunization schedules.
• Facilitate access to preventive care for members under 21.
• Provide education materials and training to members and providers.
• Conduct field work and community outreach to raise EPSDT awareness.
