Job summary

Golden Valley Memorial Healthcare in the United States seeks an experienced LPN to join our care coordination team. You will collaborate with providers, care teams and patients to support high-risk individuals across the continuum of care and facilitate safe hospital-to-home transitions.

Qualifications

• Current LPN licensure required.
• Experience in healthcare, care coordination, or population health preferred.
• Current BLS certification preferred.
• Continuing education and competency requirements maintained as required.

Responsibilities

• Collaborate with providers and care teams to identify and support high-risk patients across the care continuum.
• Meet hospitalized patients to assess discharge needs and facilitate safe transitions of care.
• Conduct readmission risk assessments and follow-up evaluations within 30 days of discharge.
• Assist in identifying patients at risk and implement strategies to reduce readmissions.
• Perform post-discharge outreach calls to assess status and coordinate follow-up services.
• Maintain timely, accurate communication with patients, teams, and stakeholders.
• Manage daily worklists and care coordination activities with proper documentation.
• Support compliance with Rural Health Clinic and Joint Commission standards.
• Participate in payer-sponsored quality initiatives and educational programs.
• Document care coordination activities in the EHR and other systems.
• Assist with quality improvement to enhance outcomes and population health.
