Job summary

Infinite Medical P.C. in Vinings, GA is seeking a responsible Licensed Practical Nurse to assist with reviewing patient charts, ensuring accuracy, and reflecting treatment requirements in EMR.

You will conduct comprehensive patient assessments, provide case management, monitor progress, collaborate with interdisciplinary teams, and maintain accurate documentation to reduce gaps and hospitalizations.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute care or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.

Responsibilities

• Provide case management services to ensure continuity of care and appropriate utilization of resources.

Schedule:

Full-Time; 9:00 am - 5:30 pm

Salary:

$30.00 an hour- $34.00 an hour

Job Summary
