Job summary

Infinite Medical P.C. in Hollywood, FL is hiring a Licensed Practical Nurse to review patient charts and ensure accurate medical records reflect treatment needs. You will assess patients, support case management, and collaborate with a multidisciplinary team to optimize care.

The position is full-time, in-office, with a schedule from 9:00 am to 5:30 pm. LPN licensure is required and at least 6 months of direct patient care experience is preferred.

Qualifications

• LPN license is required.
• Minimum 6 months of direct patient care experience in acute or managed care settings preferred (hospital, nursing home, home care).
• Proficient in patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills for collaboration with healthcare professionals.
• Experience with geriatric behavioral health is preferred.
• Ability to perform computer work for extended periods and use EMR systems.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management to ensure continuity of care and appropriate resource use.
• Monitor and evaluate patient progress; adjust care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.

Job Summary

We are seeking to hire a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient's charts. You will review patient records to ensure that information is accurate, and that records and documents accurately reflect the conditions and treatment requirements of each patient.

Requirements

• LPN license required

• Proficient in conducting patient assessments and monitoring vital signs
• Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals
• Experience with Geriatric behavioral health preferred
• Computer work including keyboard and mouse use for extended periods. Lifting/moving patients. Prolonged sitting.
