Job summary

Hamilton Health Center in Harrisburg, PA is seeking a Care Coordinator to lead a patient-centered care team and improve preventive care utilization while reducing ER visits and hospital readmissions.

The role involves co-managing high-risk patients, including home and hospital visits, educating patients and families, ensuring HIPAA compliance, documenting progress, and preparing monthly reports. The schedule is Monday to Friday, 8 am–5 pm with no weekends or holidays.

Qualifications

• Minimum: High School Graduate or GED.
• Graduate of an accredited LPN program with PA license.
• Current Pennsylvania LPN license to practice.

Responsibilities

• Provide service coordination involving case management, client advocacy, and goal-oriented action plans for patients.
• Maintain interactions with patients/families to ensure adherence to treatment plans and foster self-management and shared decision-making.
• Act as a liaison for patients regarding medical and dental benefits.
• Monitor care delivery by documenting progress, identifying barriers to care, and maintaining communication within the Patient-Centered team.
• Ensure compliance with HIPAA regulations and protect confidential information.
• Collect and utilize data for improving service activities, preparing monthly reports.
• Perform additional job-related duties as assigned.

About the Position

The Care Coordinator position promotes collaboration among the patient-centered care team to ensure quality care, increase preventive care utilization, and reduce emergency room visits and hospital readmissions. This position is responsible for directing a care coordination team supporting providers, including a Patient Navigator, Unit Clerk, Medical Assistants, Pharmacy, and Medline Team. The role of Care Coordinator, however, is not limited to: co-managing high-risk patients through assessments and care coordination, including home and hospital visits as required, and educating patients, families, and caregivers on the Patient-Centered Care Model. The role is primarily Mondays through Fridays, 8 am-5 pm, with no weekends or holiday requirements.

What You'll Do

Minimum Education/Certifications

• High School Graduate or GED Required
• Must be a graduate of an accredited licensed practical nursing program.
• Completed state board with a current license to practice as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.

Minimum Work Experience

• At least two years’ experience working with patients and families in a Community setting, Public Health, Outreach, Social Work or related field with progressively more responsible leadership experience required.
• Prior experience with care coordination/care management in a clinical setting
• Proficiency in basic data entry and computer skills.

Preferred Qualifications

• Familiarity with The Joint Commission's regulations
• Bilingual in Spanish, Nepali, or French,
