Job summary

Cardiovascular Institute of the South invites a licensed practical nurse to join Cardio@Home, coordinating care for chronic cardiovascular patients and supporting remote patient monitoring. You will collaborate with CIS physicians, hospital partners, and care teams to ensure high-quality outpatient care and program adherence.

The role emphasizes education, data reporting, and participation in quality improvement, with daytime hours and occasional evenings or weekends.

Qualifications

• Strong clinical knowledge and ability to assess cardiovascular patients.
• Excellent verbal and written communication with patients and teams.
• Highly organized with multitasking and prioritization abilities.
• Proficient in Electronic Medical Records and related software.

Responsibilities

• Serve as liaison among patients, CIS physicians, NPs, staff and hospital partners to support outpatient care.
• Coordinate Cardio@Home service lines, remote monitoring devices, dashboards and new initiatives.
• Monitor dashboards to track patient status and ensure continuity of care.
• Review medical records, analyze care delivery and adhere to CHF/AMI protocols.
• Maintain databases to support reporting, analysis and quality improvement.
• Conduct chart reviews and ensure timely, appropriate care per program criteria.
• Provide education and training to CIS staff on Cardio@Home programs.
• Assist in presenting quarterly data for quality improvement and planning.
• Participate in community outreach and marketing for Cardio@Home.
• Support business development to expand remote monitoring and telehealth programs.
• Perform outreach and schedule clinic visits to ensure follow-up.
