Responsibilities

• Behavioral Health Screening & Assessment: Administer PHQ-9, BIMS, and other screenings; Document findings in EMR; Identify changes and elevate concerns.
• Clinical Support: Gather resident history, current symptoms, and observations; monitor medication effects and mood changes; participate in care planning.
• Regulatory & Quality Compliance: Ensure screenings are completed within regulatory timelines; maintain compliant documentation; assist in quality initiatives.
• Facility & Provider Support: Serve as a behavioral health liaison between psychiatric team and facility staff; educate staff on screenings; coordinate caseloads.
