• Active LPN license in good standing.

• Strong communication, assessment, and documentation skills.

Responsabilités

• Behavioral Health Screening & Assessment: Administer PHQ-9, BIMS and other screenings; document findings in EMR.
• Clinical Support: Gather patient history, monitor mood changes, assist in evaluations.
• Regulatory & Quality Compliance: Ensure screenings meet timelines and maintain compliant documentation.
• Facility & Provider Support: Serve as liaison between psychiatric team and facility staff; assist in coordination.
