You will conduct standardized screenings (PHQ-9, BIMS) and assist in care planning, working with psychiatric providers and interdisciplinary teams to ensure compliant, person-centered resident care. Responsibilities include documenting findings in EMR, following regulatory timelines, and supporting collaboration between psychiatric staff

Qualifikationen

• Active LPN license in good standing.
• Experience in skilled nursing, geriatrics, behavioral health, or mental health preferred.
• Strong communication, assessment, and documentation skills.
• Ability to work independently in a fast-paced facility.

• Behavioral Health Screening & Assessment: Administer PHQ-9, BIMS, and other screenings and document findings.
• Clinical Support: Gather resident history and observations to support evaluations and follow-up.
• Regulatory & Quality: Ensure screenings meet regulatory timelines and maintain compliant documentation.
• Facility Support: Collaborate with psychiatric team and facility staff to coordinate care.
