Phoenix House of New York | Long Island is seeking a Licensed Practical Nurse to support clients’ health, safety, and recovery in a behavioral health and substance use treatment setting. You will monitor health needs, support medication processes, document care, and coordinate medical follow-up to foster a safe, structured healing environment.

The role emphasizes dependability, detail orientation, and empathy while adhering to policy and delivering dignified care.

Qualifikationen

• At least three years of relevant nursing experience.
• Experience with chemically addicted patients and withdrawal care.
• Current NYS LPN license; CPR/First Aid training.

Aufgaben

• Provide direct nursing care to clients under supervision of clinical leadership.
• Maintain patient confidentiality per Phoenix House policies.
• Oversee unlicensed assistive personnel (CNAs, medical assistants, technicians).
• Perform routine nursing procedures and administer prescribed medications per NYS scope of practice.
• Document vital signs, weight, and clinical assessments; report findings.
• Observe changes in patient health and communicate with clinical leadership.
• Educate patients on treatment plans, medication adherence, and self-care.
