Job summary

Connections Health Solutions in Chantilly, VA, seeks an LPN for PRN/Pool coverage to deliver comprehensive behavioral health care, administer medications, transcribe orders, and perform assessments for youth and adults.

You will collaborate with RNs, support crisis intervention, and educate individuals and families about diagnoses and medications, enhancing treatment adherence and outcomes.

Qualifications

• Graduation from an accredited Nursing Program.
• Unrestricted LPN license through Virginia Board of Nursing or multistate licensure.
• At least 2 years of psychiatric/behavioral health experience.
• Must be able to perform Safe Clinch patient restraint techniques.
• Vaccination and background check requirements per policy.

Responsibilities

• Deliver medications and document administration and transcribe orders.
• Collaborate with RN on care plans and share health data like glucose and CIWA/CINA scores.
• Provide crisis intervention and de-escalation as needed.
• Conduct assessments, collect health history, and record behavioral data.
• Educate patients and families about diagnoses and medications.
