Mayo Clinic's GIM- Ambulatory Breast Diagnosis Clinic is seeking a Licensed Practical Nurse (LPN) to provide direct patient care and coordinate care in a high-volume ambulatory setting in Minnesota.

You will conduct patient visits, perform chart reviews, and coordinate information with a multidisciplinary team. A current LPN license with BLS is required; daytime shifts with flexible scheduling are offered.

Qualifikationen

• Graduate of a practical nursing program and licensed as an LPN in the applicable state.
• One year LPN experience in ambulatory care preferred.
• Experience working in a team environment; proficient with computers and electronic medical records.
• Ability to work daytime hours with flexibility to staffing needs.
• Current LPN license; BLS certification required; additional specialty training as needed.

Aufgaben

• Conduct patient visits, review health history, and collect vitals as per protocols.
• Complete pre-visit chart reviews to ensure readiness and coordinated care.
• Perform telephone encounters to assess needs and coordinate care.
• Collaborate with providers, RNs, desk staff, and interpreters to maintain patient flow.
• Serve as a resource to the care team and facilitate next steps in patient care continuum.
• Coordinate collection of outside medical records and diagnostic information.
