Qualifications

• Graduate of a school of practical nursing and eligible for LPN licensure.
• One year LPN experience in ambulatory care preferred.
• Experience working in a team setting and with electronic records.

Responsibilities

• Conduct patient visits, review health history, and collect vitals per protocol.
• Perform pre-visit chart reviews and coordinate care appropriately.
• Handle telephone encounters to assess needs and coordinate services.
• Collaborate with providers, nurses, staff to support patient flow.
• Participate in care planning and communicate plans clearly.
• Coordinate outside records and diagnostic information for evaluation.
• Uphold patient-centered care and Mayo Clinic policies.
