PACS is seeking an LPN to provide direct patient care in a skilled nursing facility. You will administer medications, monitor vitals, and support the nursing team to ensure high‑quality care and regulatory compliance.
Responsibilities include daily living assistance, care plan collaboration, and accurate documentation. Oregon licensure and CPR/First Aid are required, with experience in long‑term care preferred.
Job Summary: The Licensed Practical Nurse is responsible for providing direct patient care and supporting the nursing team in a Skilled Nursing Facility. The LPN will deliver compassionate care to residents, assist with daily living activities, administer medications, monitor vital signs, and collaborate with the healthcare team to ensure high-quality care. This role is essential in maintaining a safe, comfortable, and caring environment for residents, while ensuring compliance with regulatory requirements.
Valid and current LPN license in the state of Oregon. CPR and First Aid certification required (or must be obtained upon hire).
Prior experience in a skilled nursing or long‑term care setting preferred but not required. Experience with PCC is a plus.
Strong communication and interpersonal skills with the ability to interact effectively with residents, families, and healthcare team members. Ability to work in a fast‑paced environment, managing multiple tasks with attention to detail. Compassionate, patient, and empathetic toward residents and their families. Basic computer skills for documentation and communication.
Ability to stand, walk, and sit for extended periods of time. Ability to lift or move residents, equipment, or supplies (up to 50 lbs). Ability to respond to medical emergencies quickly and effectively.
