Job summary

People, Incorporated, a human service agency serving the Southcoast community, is seeking a Licensed Practical Nurse to work under the supervision of the Registered Nurse in Adult Day Services programs in Massachusetts. Responsibilities include administering medications, coordinating care with health professionals, and monitoring and documenting health status.

The role requires MA LPN licensure, current CPR/First Aid on file, 2+ years in human services, and a valid driver’s license with

Qualifications

• MA LPN license is required.
• Current CPR and First Aid certifications must be on file.
• Two years of experience in human services preferred.
• Criminal background check, driving record, and valid driver’s license required.

Responsibilities

• Administer medications and treatments as prescribed during program hours.
• Coordinate each individual’s care with other health care professionals and related stakeholders.
• Monitor health status and document findings in the individual’s record.
• Document medical conditions or findings every six months as delegated.
• Report changes in condition to the RN or to the Program Director and PCP as applicable.
• Ensure oversight of PCP recommendations and interdisciplinary team therapy as delegated.
• Provide training to staff on medical protocols as delegated.
• Perform other duties as assigned by supervisor or division management.
• Attend staff meetings and required trainings including in-service sessions.
• Maintain all program and agency required trainings.
