Qualifications

Essential Functions

  • Conducts remote preadmission discussions with patients and families to capture and document patient stories and schedules visits based on complexity and urgency.
  • Enters nursing-related information into the electronic medical record (EMR), including allergies and pertinent diagnoses.
  • Triages incoming phone calls to answer nursing questions and prioritize admissions, ensuring patients with the greatest needs are escalated.
  • Makes follow-up calls and performs remote chart reviews for hospitalized patients awaiting discharge to coordinate post-discharge arrangements such as medications, DME, transportation, and other needs.
  • Supports Admissions Nurses by ordering equipment, supplies, and medications.
  • Available to cover admission and follow-up visits based on staffing and referral volumes.

Qualifications

  • Graduate of an approved school of practical nursing.
  • Maintains current Pennsylvania nursing license.
  • At least one year of nursing experience as a licensed practical nurse within the last four years.
