Essential Functions
- Conducts remote preadmission discussions with patients and families to capture and document patient stories and schedules visits based on complexity and urgency.
- Enters nursing-related information into the electronic medical record (EMR), including allergies and pertinent diagnoses.
- Triages incoming phone calls to answer nursing questions and prioritize admissions, ensuring patients with the greatest needs are escalated.
- Makes follow-up calls and performs remote chart reviews for hospitalized patients awaiting discharge to coordinate post-discharge arrangements such as medications, DME, transportation, and other needs.
- Supports Admissions Nurses by ordering equipment, supplies, and medications.
- Available to cover admission and follow-up visits based on staffing and referral volumes.
Qualifications
- Graduate of an approved school of practical nursing.
- Maintains current Pennsylvania nursing license.
- At least one year of nursing experience as a licensed practical nurse within the last four years.
