Full Time LPN (rotating weekends) to join our Transitional Care/Rehab Unit, including a $5,000 Signing Bonus.

As a non-profit 501(c)(3), Kline Galland employees are eligible for the Public Service Loan Forgiveness Program, offering relief on student loans.

We are looking for an experienced Licensed Nurse to coordinate admissions into our Transitional Care Unit. Strong organizational skills, excellent customer service, and the ability to work well with an interdisciplinary team are essential. Prior knowledge of Point Click Care (PCC) is a plus.

• Screen and process all referrals for admission to the TCU from hospital discharge planners, families, and KG communities.
• Manage all inquiries promptly with appropriate responses.
• Evaluate and coordinate admissions based on bed availability.
• Verify insurances and obtain pre-authorizations prior to admission. Advocate for skilled level of care if a member is denied.
• Gather all necessary medical information, including medication orders and special accommodations.
• Provide admission information to relevant departments and medical providers.
• Maintain census within budget and maximize reimbursement per admission, effectively managing occupancy.
• Knowledge of Medicare (federal and state regulations), HMO plans per KG contracts, and PDPM is a plus.
• Maintain relationships with hospital case managers and HMO case managers.
• Deliver excellent customer service, building relationships with care partners and families.
• Exhibit excellent verbal and written communication skills.
• Promote positive relationships among staff, residents, and the interdisciplinary team.

Qualifications

• Current LPN license in Washington State.
• At least two years of nursing or case management experience.
• Proficiency in computers, teamwork, customer service, and multitasking.
• Experience in multicultural settings is preferred.
